Hello.
First, sorry about my bad english. I come from germany. icenhancer 1.35 working not good with my GeForce GT 530. I have 10-15 fps and this is very bad. Must i buy a new and better video card?

Mostly, yeah, a better GPU might help you to get better FPS. The CPU too, you need both to be good.
Also try to find in iceconfig.ini
EnableAmbientOcclusion=true
Put it to false

Hmm ,Are you sure the GTA IV is running on Nvidia Gpu?,Optimus problem ?

Try closing all the unnecessary processes. Using Gamebooster 3.0 fixed my lag problems. So try using this program. Also make sure you are using up-to-date drivers and not some broken, outdated drivers.

reducing the settings to low and using the low - end might help you and as dmcprince1 said if you use Gamebooster 3.0 also, you might get about 25 - 30 fps if I'm right.
