Hm, which card are you using? If you are using and ATI card, try setting ForceNvidiaCard=true to ForceNvidiaCard=false.
Try reinstalling IV completely (deleting whole GTA IV folder after uninstall).
You could also update/reinstall your drivers ( GPU, Direct X ect.)

Rent annother GPU from a friend or someone else. Maybe your GPU is damaged (hopefully not).

Use drivers sweeper to clean drivers.
Reinstall last stable drivers for you.
As said, try other GPU.

It's from GPU.I asked few peoples who are repairing GPU's and they test it and all said it's from GPU.
