Thank you SO much for this thread. This makes the 4th day I've been trying to get Icenhancer to work. I've been experiencing crashes due to GTA refusing to recognize my graphics card. It would give me the ol' 54MB of memory and never load when I start the game. This thread helped me get past that but I still have a lot of problems.
I have a rather powerful laptop: ASUS i7-2630QM @ 2.00GHz nVidia GeForce GT 540M 1GB 8GB RAM
The problem I have now is when I get in-game, the lighting is absolutely ridiculous. The dark is too dark, and the lights in the dark are too bright. During the day, everything looks bleached with light. I have tried numerous setups and configurations, but this still stays the same.
Should I use a different file other than d3d9.dll? I have EnableProxyLibrary and such enabled in iceconfig pointing towards d3fx.dll. I have xlive in the folder, the version included from the latest official release. Is it worth noting that I have to do some crazy things to get it to display at the correct resolution, like Alt+Tabbing away and back and moving the screen and changing resolutions back and forth.
I've gone entirely too far to give up on this, however I feel lost now having exhausted every option I see in front of me. I would appreciate any help.

Which version of GTA IV are you on? Change the proxy library back to smaa.dll (ProxyLibrary=SMAA.dll)

Thank you very much for this! I can confirm this works on a laptop with Nvidia GTX 675M Core i7-3610QM 8GB RAM However the CPU is bottlenecking the GPU, and I only get around 50%-70% GPU usage, which only gives decent performance at lower settings and resolutions(1366x768). Also upon lowering the resolution or changing settings in game, the FPS decreases from around 20-25 to around 11-13. Nevertheless I am very happy that it works on Optimus

I am also trying to make Icenhancer work with EFLC. I changed the exe file and renamed the dll file. But error keeps popping up whenever i try to start the game. Btw i have xlive.dll also present.

Thanks for your guide, it solves the problem, but you said it could bring down FPS's. On my laptop (i5 3210, 4GB RAM, GT630M 2GB) it totally cuts to few frames per second. Normal GTA IV worked absolutely fine, now colors are much better, but it's useless I was using ICE on older conf (E6550, 2GB RAM, HD2600XT 256MB!) and it didn't cut FPS perceptible. Is there any way to fix it or I need to uninstall ICE?
