Always use 1.0.4.0 if possible, it's the best patch for ENB/Graphic mods for GTA IV allowing it to use all effects of the mod.

If you delete the d3d9.dll you'll remove iCEnhancer...
I see. So the problem wasn't because of the crack, it was because of iCEnhancer. If you want to use iCEnhancer copy & paste the main files from the mod into the main GTA IV folder again. Be sure to install xliveless too.
Also close all background programs running. They might interfere with iCEnhancer.

Here, rapidshare.com/files/484540237/GTA IV 1.0.4.0.7z The patch for 1.0.4.0 is included too. Just place LaunchGTAIV.exe & 1911.dll into the Main GTA IV folder. Start GTA IV through LaunchGTAIV.exe as admin.

I suggest checking your temperature during playing then, you PC could be overheating...
Start GTAIV through LaunchGTAIV.exe as admin. That might solve it.
You don't have any other mods besides iCEnhancer installed, right?

Uninstall GTA IV again, be sure to delete the Grand Theft Auto IV folder too. Then install it again. Install iCEnhancer 2.1 by copying all files from "Main Files" into the main GTA IV root folder. Then install the shaders too, located in "2 - shaders" in the 1.0.7.0 - EFLC folder. Be sure to read the readme there.
It should work now, you can install xliveless if it still doesn't.

Could you please tell me your PC specs please?
Close all background programs, including MSI Afterburner, incase some BG program is causing that issue.
Try lowering your In-game settings. You could try this too: Create a textfile called commandline and write this in the file: -noprecache -nomemrestrict -norestrictions -availablemem 2.5
Save the file & put it in your GTA IV root.

Try cacheboost: www33.zippyshare.com/v/64239231/file.html(Credit goes to legaccy again) If this doesn't work try creating a textfile. Name it commandline and write this in the file: -Noprecache -nomemrestrict -norestrictions -availablevidmem 2.5 Save it & put it in your GTA IV root folder. Start GTA IV through LaunchGTAIV.exe as admin & see.

You could just install both GTA IV & EFLC and also install the same enb on both of them. You'll need to patch GTA IV to 1-0-4-0 if you have the CD version, EFLC is 1.0.7.0 by default.
You could install a Simple native trainer (how to install & use on YouTube), to make the time run faster & more.

You could get new scripts/mods like: First person mod, realistic gun sounds, VIVA New York/LN's Time Square textures, Realistic driving & Flying + new weapons. As Aksine said, you can find most of them on GTA4-mods & alot on gtainside too. Entering some mod names into google should help you finding them too.

Use a backup or reinstall GTA IV completely.
After reinstallation, downgrade & patch to 1.0.4.0. Install iCEnhancer 2.1 Main files located in iCEnhancer2_1FINAL folder/1.0.3.0 - 1.0.4.0 - 1.0.0.4/iCEnhancer 2.1 files.
Also install the content from "Only if fresh install" folder (copy & paste both xlive.dll & xlive_d.dll into the main GTA IV folder).

Hey there, first, i suggest installing cars manually, maybe you heard of this method. Most car packs are pretty bad, cause bugs & some weird stuff. If you still want to use that car pack try this: 1. Delete the currently using scripthook & asiloader. 2. Download this and place all 3 files into your GTA IV root folder. 3. Start the game through LaunchGTAIV.exe as admin.

Try it first with current GPU. If it doesn't run that well on med setts, play on lowest settings (both GTA IV Graphic options & iCEnhancer).
